Safety Incident Report:

The coach or manager should complete an incident report every time a player is injured while participating in a Rockford Little League event.

bulletLook your forms over well and fill them in as completely as possible. Over time you can forget the particulars of the incident and the report will be the only document that can trigger your memory.
bulletOf special attention should be the area where you describe what happened. This should be written objectively as if you are giving data only. It should never be accusatory, biased, or blaming.
bulletThe parent contact area also needs to be filled in completely. Use quotations if necessary stating parents wishes, such as: "I do not want you to call an ambulance" or "I don’t think he needs any treatment" and document parents name who made decision.
bulletIf you as a coach or manager feel child should be seen and said this to parent who did not agree, document that you suggested this to parent and that they declined.

All completed incident reports need to be turned in to the Safety Officer as soon as possible.
